On each of the above dates we shall be at your chosen place to manufacture and fit an ocular prosthesis in your presence. We look forward to having the pleasure of welcoming you on one of these dates.
If you are a member of a statutory health insurance fund in Germany you will need a doctor's prescription with details of the respective diagnosis, besides which you have to make an additional payment of 10 euros.
If your cost-bearing body is an employer's liability insurance association or an orthopaedic supply centre you will need an order from the respective cost-bearing body.
If it should not be possible for you to appear in person or if you need a further ocular prosthesis as replacement or spare, you can send us a well-fitting sample – carefully packaged – to our address in Würzburg.
If your address changes, please info [at] augenkuenstler.de (subject: Move, body: My%20address%20has%20changed.%20It%20now%20reads%3A) (notify us) accordingly so we can continue sending you our information, news and messages. As state certified master craftsman's business we have accreditation with all health insurance funds and authorities.
